FT, minimum three years comprehensive related experience. Non-profit organization seeks individual with prior experience working with consumers to understand and impact key issues related to successful aging including; nutrition, medication management, adherence to medical provider’s plan of care, importance of established supportive care from various systems including homemaking, personal care, transportation, nutrition, therapies. Assist consumers in developing & implementing an action plan to access necessary resources that aid the client in remaining at home. Requires productive knowledge of health related issues & in-home based supportive services. Must enjoy working collaboratively with consumer, health care team & community resources. Abundant working knowledge of issues related to aging; community resources, health care professionals/systems, insurance. Solid interpersonal & organizational skills. Strong initiative & judgement abilities; compassionate, analytical, & comprehensive approach to working effectively with consumer & key care-givers including: health professionals, community resource providers, families. Must be a creative & effective problem solver who strives for excellence and seeks positive & proactive solutions. General office skills also required including computer. Background check will be conducted. Must have own means of transportation for work related travel within metro area and foothills. Ability to speak Spanish a plus.
