Care Management Services
Seniors'
Resource Center's Care Management Services provides community
education, emergency financial assistance, information and referral
services, care management, Friendly Visitors and supportive counseling
to hundreds of seniors. Professionals can assess simple situations
over the telephone, and may make a home visit when the needs are
more complex. They answer questions about many subjects and direct
people to appropriate resources.
One of SRC's primary functions is care management. The first
step in providing this service is a good assessment. Our Professionals
are often asked to intervene by clients or their families when
a senior's level of functioning has deteriorated. They will evaluate
the amount of support available to the client and develop a care
plan that will meet the needs of both the client and their family.
This is provided for a modest fee, based on the client's income.
Care managers strive to maximize independence, promote self-determination
and advocate for services for their clients. Through services,
advocacy and resource development, care managers help families
achieve the highest quality of life possible.
You can contact care managers from 8 a.m. to 5 p.m., Monday through
Friday at 303-235-6923. The program is located at 3227 Chase Street
in Wheat Ridge.
