Information and Referral
Offers
comprehensive care management including assessment and program/agency
referral, emergency assistance, counseling, and educational/social
group interaction to scheduled or walk-in participants.

Adult Day and Respite Services
Provides a state certified, therapeutic environment of socialization
and stimulating recreational activities for adults requiring supervision
and assistance.

and positive attitude go a long way toward brightening the long
and difficult days of seniors, older adult car giver."
In-House Care Services
Insure continued independence in the home for seniors and disabled
adults, through the services of trained providers of personal
care and homemaking services.
"My provider not only helps me keep my house in order, she
has become a real friend to me, In-House care client."
Transportation Services - Evergreen
Facilitates rides for community members to shopping, appointments,
activities, nutrition programs, etc. Schedules include weekly
trips to Denver and Conifer.
"Since I can't drive anymore and I don't have family here,
the Transportation Services bus keeps me from being a hermit,"
SRC-Tran.
Nutrition
Promotes quality dietary intake to area seniors with homecooked,
noon time meals offered twice weekly in the Yellow House Cafe.
"Meals on Wheels" meals are prepared and delivered daily
to homebound seniors. Additionally, food and goods are assembled
and distributed every other month to those in need.
"I have such fun at the cafe. I get to eat a good meal,
visit with friends and stay for entertainment or a program in
the afternoon," faithful Evergreen driver.
Activities
Encourages social interactions and community involvement through
attendance at a variety of programs including arts, crafts, movies,
travelogues, shopping, entertainment, cultural visits, etc.
"There are so many things to do through the Yellow House.
I can go shopping and go to programs in Denver, and visit with
my friends on the bus instead of driving," Senior Resident
of Conifer.
Volunteer Opportunities
Foster many options for volunteers of all ages to assist SRC-Evergreens
staff and customers including volunteer driving, adult day &
respite services and the Yellow House Cafe, administrative support
and special event coordination.
"Working as a volunteer provides me with the personal reward
of giving back to my community and helping seniors whose lifetime
contributions have been invaluable," SRC-Evergreen Volunteer.
